APPLICATION FOR REGISTRATION
DENTAL HYGIENIST

The following is required for licensure:

1. Completed notarized application form [ www.nbdental.com/forms/Eng_DH_app.pdf ] (Acrobat PDF format). Please make sure your application is notarized by a commissioner for oaths or notary public (lawyer).

2. A copy of your diploma from a school of dental hygiene approved by the Commission on Dental Accreditation of Canada. (New grads: The Society will accept a letter from the Dean of the College confirming your graduation from the dental hygiene program. A copy of your diploma is to be provided at a later date when received from the school).

3. Please provide a letter of good standing from the Provincial Licensing Board if you are licensed to practise in an other province.

4. A cheque made payable to the New Brunswick Dental Society for registration fee of $340.00. This amount represents a one time registration fee of $40.00 and your annual license fee of $300.00. Credit Cards not accepted. Annual fees are not pro-rated or refunded. Licences are valid for the calendar year January 1st to December 31st.

5. Mandatory Continuing Education - Licensed Dental Hygienists will need to obtain a minimum of 36 credit hours over a three year period and shall be reported to the New Brunswick Dental Society.

Dental Hygienists applying for an initial license or a renewal of license to practise in New Brunswick will have to meet a pro rated level of credits for continuing education that will be calculated on the basis of how many months of licensing time remains available in the cycle of CE recording from the time of issuing of the license.
