Application for Registration : Dentists
The following is required for licensure in the Province of New Brunswick:
1. The completed notarized application form [ www.nbdental.com/forms/dentist_app_eng.pdf ] (Acrobat PDF format).
2. A photocopy of: Graduation diploma from a school of dentistry (new graduates may complete section 13 of the application and supply a copy of the diploma at a later date when received from the school). National Dental Examining Board of Canada [ www.ndeb.ca ] Certificate (new graduates may submit an official letter from the NDEB if their certificate has not yet been received).
3. Proof of malpractice insurance to the minimum amount of three million dollars is required. Malpractice insurance is available by calling Canadian Dental Service Plans Inc. (CDSPI) at 1-800-561-9401. CDSPI will put a binder on for the coverage which will become effective the date of licensure.4. If currently or previously registered in another province, the applicant must complete section 14 of the application form which certifies the applicant is in good standing at the present time OR provide a letter of good standing from the province in which the applicant is currently or previously licensed.
5. Applicants who wish to obtain a license as a Specialist must provide proof of having successfully completed the National Dental Specialty Examination (NDSE)6. The fee to be remitted for 2010 is $2650. This amount represents a one time registration fee of $150.00 plus the annual license fee of $2500. No credit cards accepted. Annual fees are not pro-rated or refunded.
7. Forward the completed application form with required documents and a cheque for the license fee to the New Brunswick Dental Society. Please include a phone number where you can be contacted in order to make an appointment to meet with the Registrar at the Society office in Fredericton at which time you will receive your license to practice.
