Application for Registration : Dentists 3. Proof of malpractice insurance to the minimum amount of three million dollars is required. Malpractice insurance is available by calling Canadian Dental Service Plans Inc. (CDSPI) at 1-800-561-9401. CDSPI will put a binder on for the coverage which will become effective the date of licensure. 4.
If currently or previously registered in another province, the applicant
must complete section 13 of the application form which certifies
the applicant is in good standing at the present time OR provide
a letter of good standing from the province in which the applicant
is currently or previously licensed. |
